Is your child is up to date on immunizations?

Records are usually kept by the physician’s office that administered the vaccines.  Information may also be found by calling the Public Health Office.

What immunizations are required for school?

State requirements for admission into KINDERGARTEN are:

3 doses of Polio vaccine with one dose given after the 4^th birthday,
5 doses of DTaP with one dose given after the 4^th birthday
2 doses of MMR
2 doses of Varicella (Chickenpox)

State requirements for admission into 7^TH GRADE are:

Above Kindergarten requirements have already been met

AND

1 additional dose of TdaP for the entry into 7^th grade

Immunizations are given Monday – Friday 8:00 am – 4:30 pm, by appointment!
